My Health Toolkit® is a one-stop destination for managing your benefits — customized just for you!
Within My Health Toolkit, you can learn more about your coverage, check medical and dental claims, manage your prescriptions, access your identification card, and find a doctor or hospital. You also have access to Rally®, a digital platform that can help you improve and maintain your overall health. Rally is a product of Rally Health Inc., an independent company that provides a digital health platform on behalf of the State Health Plan.
It’s easy to sign up!
In just a few clicks, you’ll have everything you need at your fingertips.
- Select Register Now in the Member Login box on the right side of this page.
- Enter your member identification number on your State Health Plan identification card and your date of birth. If you don’t have your insurance card, enter the Social Security number for the State Health Plan member and your date of birth.
- Choose a username and password.
- Enter your email address and decide if you’d like to go paperless.
If you have not created an Express Scripts account, you’ll be prompted to create one the first time you access your pharmacy benefits through My Health Toolkit. Express Scripts is an independent company that contracts directly with the State Health Plan.
