Our wellness programs are here to help you get on the right track for achieving better health. When you enroll, we’ll connect you with a health coach who will help you create a personalized action plan for meeting your goals. You’ll also receive educational tools and resources by mail.

How to Enroll 

If you are a member of the State Health Plan who could benefit from one of our wellness programs, you may have already received a letter in the mail with instructions to complete a short survey. There are also two ways to self-enroll:

  1. To enroll online, log in to your My Health Toolkit® account. Select Wellness, then Personal Health Record. From your Activity Center, complete the health assessment specific to your condition.
  2. Or, call 855-838-5897.

Healthy Weight for Kids and Teens

Healthy Weight for Kids and Teens is for overweight and obese dependents between the ages of 2 and 17 years. If we identify your child as someone who could benefit from the program, you will receive an invitation by mail to participate. When you enroll, you will receive a parent’s guide and educational materials for your child tailored to his or her age group. We’ll assign you a personal health coach who can provide education and support to help you and your child overcome barriers to living a healthy lifestyle.

Weight Management for Adults

Enrollment in this program is available to any eligible member age 18 or older. The Weight Management program assists members in determining a personalized plan for lifestyle modifications to better manage their weight. The program educates members about evidence-based guidelines for healthy eating and exercise as well as strategies to maximize weight loss and maintenance.
