Dental Coverage

Your dental benefits are divided into four classes. We allow benefits based on the Basic Dental Fee Schedule. Please note we do not cover all dental services under this plan. Refer to the Exclusions section of your Insurance Benefits Guide.

Diagnostic and Preventive Services (Class I)

• Oral exams
• Prophylaxis (cleaning and scaling of teeth)
• Fluoride treatment
• X-rays
• Space maintainers (for covered persons under age 19)

Basic Dental Services (Class II)

• Restorative (fillings) procedures
• Extractions
• Periodontal (gum) procedures
• Endodontic (root canal) procedures

Prosthodontics (Class III)

• Crowns
• Dental implants
• Bridges
• Dentures
• Onlays

Orthodontics (Class IV)

• Limited to covered children under age 19
• Correction of malocclusion (including diagnosis, models and radiographs)
• Active treatment (including necessary appliances) 

Dental Plus Network

Taking care of your teeth and gums is important for your overall health. And now, you can save money — and make the most of your benefits — when you use a dentist in the Dental Plus network. Dental Plus has higher allowed amounts, which are the maximum amounts allowed by the plan for a covered service. Network providers cannot charge you for the difference in their cost and the allowed amount.
